Qualifications

• RN required as part of the role.
• Registered Nurse Diploma required to practice.
• 1 year of experience required in case management or related field.

Responsibilities

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers to participate in treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure patient-centered care.
