Job summary

HCA Houston Healthcare Medical Center in Houston, TX is seeking a Registered Nurse Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans tailored to each patient's needs, while partnering with physicians, nurses, and department leaders to ensure safe transitions and clear communication.

Qualifications

• Five years of acute care experience required.
• Two years of utilization review/case management experience.
• One year experience in discharge planning in acute care setting desirable.
• RN license in Texas is required.
• BSN preferred; master's degree desirable or commensurate experience.

Responsibilities

• Guide patients and families through program orientation and discharge planning milestones.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop individualized treatment plans reflecting patients' strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations; document findings and communicate needs.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence in every interaction.

Education

Bachelor’s degree in Nursing

Associate Degree
