Job summary

Geisinger Health Plan is hiring an RN Case Manager to coordinate patient-centered care for commercial populations. The role emphasizes collaboration with a multidisciplinary team and adherence to utilization management, quality and fiscal outcomes.

The position requires at least 2 years of RN experience, case management familiarity, and preferably ACMA certification and a BSN. Work is full-time, days, in a salaried role within Geisinger Health Plan.

Qualifications

• At least two years of RN work experience required.
• Case management experience and ACMA certification preferred.
• BSN preferred.
• Knowledge of healthcare laws, regulations and insurance policies; proficiency in healthcare IT systems and claims processes preferred.
• Ability to work autonomously and in a multidisciplinary team.
• Active compact RN license is a plus.

Responsibilities

• Assess, plans, coordinates, monitors and evaluates care to optimize patient health status.
• Support commercial population with population health initiatives.
• Document and maintain case management activities and data.
• Collaborate with health team to ensure coordinated services.
• Apply evidence-based guidelines and protocols to care plans.
• Monitor utilization and adjust plans as needed.
