Job summary

Qualifications

• At least two years of RN work experience required.
• ACMA Certified Case Manager preferred.
• BSN preferred.
• Case management experience preferred.
• Knowledge of healthcare laws, regulations and insurance policies helpful.
• Ability to work autonomously and as part of a multidisciplinary team helpful.

Responsibilities

• Coordinate patient-centered care through case management.
• Support utilization and practice metrics to improve care delivery.
• Develop treatment plans aligned with evidence-based guidelines.
• Collaborate with patient, family, providers to coordinate services.
• Maintain documentation for case management activities.
• Monitor laboratory results and utilization patterns.
