Job summary

HCA Florida Citrus Hospital is seeking a dedicated Case Manager RN to support patients and families through the care journey. You will coordinate services, develop care plans, and collaborate with physicians and nurses to ensure safe transitions and high-quality care.

The role requires an RN license in the state where practicing, with 2+ years in case management or 3+ years in clinical nursing. InterQual and Case Management certification are preferred.

Qualifications

• Required: Associate Degree in Nursing or Nursing Diploma and RN license in the state of practice.
• Preferred: Bachelor’s degree in nursing.
• Experience: 2+ years in case management or 3+ years in clinical nursing.
• InterQual experience and Case Management certification preferred.

Responsibilities

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers for participation in treatment planning.
• Develop and implement individualized treatment plans.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with Rehab Program Director and care team to ensure seamless, patient-centered care.
• Promote HCA Healthcare values in every patient and family interaction.
