Overland Park Regional Medical Center is seeking a Case Manager to support patients and families through care journeys, coordinate services, and develop individualized care plans. Qualifications

• Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
• 3+ years of clinical nursing experience required.
• InterQual experience preferred.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and benefits.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and recovery goals.
• Document psychosocial assessments and discharge planning evaluations for care team communication.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote values of compassion, respect, and excellence in every interaction.
