AccentCare, Inc. in Brooklyn Park, MN is seeking a Registered Nurse (RN) Case Manager to coordinate and oversee patient care from admission to discharge.

This role balances direct patient care with coordination of a multidisciplinary team, ensuring the plan of care meets clinical goals and quality standards. The RN Case Manager will support a coverage area that includes all of Brooklyn Park and work within a schedule that aligns with start-of-care needs, typically Tue-Sat or Wed-Sat on 10-hour

Qualifications

• Graduate from an approved school of professional nursing and currently licensed as an RN in the operating state.
• Minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsabilités

• Assess nursing needs and manage the patient’s care plan from admission through discharge.
• Complete initial patient assessment on admission with history.
• Identify nursing diagnoses and monitor drug therapy for safety and effectiveness.
• Set measurable goals and communicate results to physicians and team.
• Provide nursing interventions within the Nurse Practice Act and re-evaluate needs as required.
• Document assessments, progress, and discharge notes promptly.
• Inform physicians and staff of changes in patient condition.
