Job summary

Supreme Touch Home Health Services, Corp. seeks a Registered Nurse to fulfill 5 to 10 visits per week or more as an RN Case Manager, delivering compassionate in-home nursing and coordinating patient care.

The RN will conduct initial and ongoing assessments, develop and modify care plans, supervise home health aides, communicate with physicians, and ensure documentation and transfer plans are completed for each patient.

Qualifications

• Graduate of an approved school of Nursing.
• Registered Nurse in the state(s) of service delivery.
• Minimum of two years of experience as a Professional Nurse in home health care setting.

Responsibilities

• Initial assessment conducted within 72 hours of patient acceptance into home health.
• Reassessments conducted every 60 days or as patient needs warrant.
• Coordinate patient care with other members of the home health care team.
• Supervise home health aides and assign duties to aides.
• Inform physician of changes in patient condition and summarize status for physician every 62 days or as needed.
• Ensure documentation of findings and ongoing services in the medical record.
• Participate in transfer, discharge planning, and discharge summaries.
• Document verbal orders from physicians in the medical record.

Education

Graduate of an approved school of Nursing

Registered Nurse in the state(s) of service delivery

Minimum of two years of experience as a Professional Nurse in home health care setting
