Job summary

Enhabit Home Health & Hospice is seeking a Registered Nurse Case Manager to coordinate patient care and oversee visit frequency across care disciplines. You will collaborate with physicians and office staff to ensure accurate data and timely lab reporting, including OASIS collection.

The role requires an active RN license, at least one year of clinical experience, and a valid driver’s license, with opportunities for continuing education and professional growth in a supportive team environment.

Qualifications

• Must be currently licensed as a Registered Nurse (RN) in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Perform visits and coordinate client care; oversee frequency of visits for the episode.
• Consult with physician and office and provide details about patient care.
• Accurately perform OASIS collection; ensure medication profiles are current and labs reported timely.
• Attend weekly case conference and monthly case manager meetings.
