Job summary

Kansas City Hospice and Palliative Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary team. You will provide high-quality hospice care, support patients and families, and collaborate with physicians, APRNs, social workers, and chaplains to ensure comprehensive, patient-centered care.

Ideal candidates hold MO/KS RN licenses (or compact), have hospice experience, and demonstrate strong communication, organizational, and autonomous work capabilities.

Qualifications

• Current, valid Registered Nurse license in Missouri and Kansas (or compact license).
• Minimum experien ce: hospice or end-of-life care preferred.
• Strong communication with patients, families, and interdisciplinary team.
• Excellent organizational skills and ability to multitask.
• Valid driver's license and reliable transportation; CPR certification required.

Responsibilities

• Provide compassionate, high-quality hospice nursing care to patients in their homes or care settings.
• Coordinate and communicate with the interdisciplinary team to plan and adjust patient care.
• Educate patients and families on care plans, medications, and symptom management.

Position Description: We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones.

The Ideal Candidate Will Possess the Following Requirements:

• Valid, unencumbered Missouri and Kansas RN or compact license to include both states
• Minimum of one year bedside experience in caring for hospice patients is preferred
• Ability to communicate effectively and professionally with others; and to prepare and present information in oral and written formats
• Superior organizational skills with the ability to multitask and meet deadlines; must schedule and manage time effectively
• Valid driver’s license and insurance plus reliable transportation
• CPR certification required
• Ability and desire to work autonomously in the field with understanding of strong team support
