Phoenix Home Care & Hospice in the Kansas City area is seeking an RN Case Manager to deliver compassionate hospice nursing care to clients and their families. You will work with the interdisciplinary team to develop and implement a plan of care that centers on quality of life.

The ideal candidate brings energy, strong managerial, organizational, and interpersonal skills, and the ability to guide patients and families through care decisions.

Qualifications

• Active Kansas or Compact RN license.
• Hospice experience preferred; training provided.
• Valid driver’s license and auto insurance.

Responsibilities

• Provide compassionate, hands-on hospice care.
• Support and educate patients and families.
• Complete assessments and ongoing evaluations.
• Collaborate with physicians and the hospice team.
• Address patients’ physical, emotional, and comfort needs.

• Pay Range: $80,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
