Job Description: The RN Case Manager provide a collaborative process of assessment,planning, facilitation, case management, evaluation and advocacyfor options and services to meet each patient’s needs to achieveoptimal clinical, financial, and satisfactory patient outcomes. Theoverall goal is to enhance the quality of patient care throughoutthe continuum of care to achieve excellence in healthcare delivery,within a framework of fiscal responsibility and focus onoptimization of the transition between the outpatient, inpatient,and post-acute settings.

Minimum Qualifications:

• 1. BSN with current RN licensure recognized by the State of Ohio isrequired.
