Job summary

Henry Ford Health is seeking a Registered Nurse Case Manager for full-time Day shifts in the Detroit area. The role focuses on care coordination, discharge planning, and advocating for resources to ensure safe transitions across the care continuum.

The ideal candidate will have 3+ years of clinical experience, excellent customer service, and proficiency with EPIC and Care Port. On-site position with opportunities to impact patient outcomes across Michigan facilities.

Qualifications

• Minimum three years of clinical nursing experience.
• Excellent customer service and interpersonal skills across all levels.
• Strong problem-solving, analytical and decision-making abilities.
• Knowledge of discharge planning and care coordination; experience with home health or managed care is preferred.
• Experience with EPIC and Care Port is preferred.

Responsibilities

• Create comprehensive, patient- and family-centered care plans.
• Monitor medical necessity and level of care through assessments.
• Advocate for resources and remove barriers to care delivery.
• Maintain dialogue with supervisors and care transition team for plan reevaluation.
• Serve as a resource for medical records documentation and post-discharge options.
