Henry Ford Health is seeking a Case Manager for on-site day shifts in Grand Blanc, MI. This role focuses on patient-centered care, discharge planning, and effective coordination across care teams to ensure safe transitions and optimal outcomes.
The ideal candidate holds an RN or LMSW license in Michigan, with 3+ years of clinical experience and proficiency in care planning, documentation, and advocacy for patients and families. Epic and CarePort experience are preferred.

Case Manager - Full Time Day
4 - 10 hour shifts w/every 4th weekend plus holiday rotation. 7:30am - 6pm. .
This is an "On-site" position.
Orientation is approx. 6-10 weeks
Role Overview
TheCaseManagerisamemberofthehospitalorspecialty-basedpatient-centeredcare team or treatment team responsible for the collaborative practices of identification, assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s health care needs. The goalsof the case manager are to promote patient safety, quality ofcareand cost-effective outcomes. The case manager provides effective discharge planning services that support a safe transition to the next level of care and addresses the needs of patients whohaveexperiencedacriticaleventordiagnosisthatrequiresmanagementstrategies to optimize health outcomes along the care continuum.

Required Qualifications
EDUCATION/EXPERIENCEREQUIRED:
Minimum(3)threeyearsofclinicalexperience.
Excellentcustomerserviceandinterpersonalskillsincludingtheabilitytointeract with internal and external customers and all levels of the organization.
Strongproblem-solving,analytical,anddecision-makingskills.
Strongcomputerskillsandknowledge. EPIC & Care Port experience preferred
Experienceindischargeplanning,homehealthcare,rehabilitativemedicine,or managed care preferred.
Knowledgeofpreventiveserviceguidelines,clinicalpracticeguidelines,behavior change theory, Medicare and Medicaidregulationsand case managementprinciples.
Knowledgeofmedicalethicsandlegalimplicationsrelatedtocasemanagement.
Understandingofsocialdeterminantsofhealthandtheirimpactonapatient’s
Wellversedinfacilitatingcommunityresourcestomeettheneedsofdiversepopulations.
Strongorganizational,planningandimplementationskillswiththeabilitytohandle multiple complex patients’ needs simultaneously.
Strongsenseofcompassionwiththeabilitytosuccessfullyadvocateforpatientsand their families.
CERTIFICATIONS/LICENSURESREQUIRED:
RegisteredNurse(RN)oraLicensedSocialWorker(LMSW)withavalid, unrestricted State of Michigan license.
Certification in Case Management (CCM) by the Commission for CaseManagement Certification (CCMC)orAccreditedCaseManager(ACM)bytheAmericanCaseManagementAssociationpreferred.
