Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager to join our on-call team for weeknights (7 on, 7 off) in Georgia. You will evaluate, plan, and coordinate hospice patient care in home or facility settings.

The role emphasizes documentation, adherence to policy, and maintaining continuity of care while delivering compassionate nursing support.

Qualifications
