Job summary

PruittHealth Hospice is seeking a compassionate Registered Nurse Case Manager (RN) to provide expert hospice care in patients' homes or care facilities. You will assess, plan, implement, and monitor individualized care plans to support comfort and quality of life for patients and their families.

Ideal candidates hold an active RN license and will follow policy-driven procedures, ensuring accurate documentation and continuity of care across settings.

Qualifications

• Current, active, and unrestricted Registered Nurse licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
