PruittHealth Hospice seeks a Weekend On-Call Registered Nurse Case Manager to join our patient-centered team. You will assess, plan, and coordinate hospice care for patients in their homes or care facilities, ensuring adherence to individualized care plans and company policies.

Qualifications
