Job summary

HCA Healthcare's Methodist Hospital Metropolitan in San Antonio seeks a Case Manager to support patients and families through their care journey, coordinating services, resources, and care plans in partnership with physicians, nurses, and leaders to ensure safe transitions.

The role emphasizes patient-centered care, documenting assessments, developing individualized plans, and collaborating with the rehab program director and care team, while upholding our values of compassion and excellence.

Qualifications

• RN license to practice as a registered nurse in the state (Texas) or compact license.
• Minimum 3 years of clinical experience in the area of case management.
• Preference for at least one year in utilization review, resource management, discharge planning or case management.

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers for treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation and cross-team communication.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for patient-centered care.
• Promote values of compassion, respect, and excellence in every interaction.
