Job summary

Qualifications

• Graduate of an accredited nursing program (dip, adn, or baccalaureate).
• Active Texas RN license or multi-state compact with Texas license within 90 days.
• Prefer 3 years of RN experience and case management exposure.

Responsibilities

• Support patients and families through care journey with coordinated services.
• Develop individualized care plans reflecting patient needs and goals.
• Coordinate education for patients, families, and caregivers on treatment planning.
• Complete psychosocial assessments and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every patient and family interaction.
