Job summary

HCA Houston Healthcare Tomball is seeking a Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s unique needs, partnering with physicians, nurses, and department leaders for safe transitions and consistent quality.

You will guide education for patients and families, develop individualized plans, and document psychosocial assessments to ensure clear communication

Qualifications

• RN (Registered Nurse)
• Associate Degree required
• No Travel Required
• 2 years experience in case management or 3+ years clinical experience

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Develop and implement individualized treatment plans reflecting patient strengths and needs
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care
• Promote HCA Healthcare values through every patient and family interaction
