Qualifications

• BSN or higher from an accredited program.
• Active, unencumbered RN license.
• 1+ year experience in case management.
• Strong interpersonal and verbal/written communication.
• Proficient with electronic health records (EHR).

Responsibilities

• Conduct comprehensive assessments of patients and needs.
• Collaborate with providers, patients, and families on care plans.
• Monitor progress and adjust care plans as needed.
• Serve as main contact between patient and providers.
