Qualifications

• BSN or higher degree from an accredited program
• Active and unencumbered RN license
• 1+ years experience in case management, preferably in healthcare/hospital
• Exceptional interpersonal and communication skills to collaborate with medical professionals, patients, and family
• Proficiency in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive assessments of patients, including medical history, medications, and psychosocial needs
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans
• Monitor and evaluate patient progress and adjust care plans as needed
• Serve as the main point of contact between patient and healthcare providers for seamless communication
• Maintain up-to-date records and case notes with confidentiality and compliance
