Hackensack Meridian Health is seeking an experienced RN Case Manager to oversee patient care for a defined group of patients. The role guides the plan of care in line with population-specific guidelines and coordinates direct care delivery through a multidisciplinary team.

The ideal candidate holds a NLN/AACN accredited nursing program, a Bachelor's degree in nursing, and at least two years of RN experience, with an active NJ RN license and specialty certification.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Bachelor's degree in nursing.
• 2+ years of experience as a registered nurse.
• NJ State RN license.
• Certification in area of specialty.

Responsabilités

• Oversees and directs patient care activities for a specific group of patients.
• Revises plan of care based on population-specific guidelines/protocols.
• Oversees direct delivery of care by the multidisciplinary team.
