Job summary

AccentCare is seeking an RN Case Manager to oversee patient care plans from admission to discharge in the Gainesville, GA area. This full-time role includes direct and indirect patient care, directing RNs and LPNs, and coordinating with CHHAs to deliver individualized care plans.

The position requires an RN license in the state, CPR certification, and at least one year of RN experience. On-call duties are included, with a typical Mon-Fri schedule and an annual compensation in the listed range.

Qualifications

• RN Case Manager is responsible for managing patients' care plans from admission through discharge.
• Coordinate interdisciplinary care and direct staff involved in the plan of care.
• Assess nursing needs, establish goals, and communicate with physicians.

Responsibilities

• Assess status of nursing needs and manage patient care plans.
• Complete initial patient assessment on admission and gather patient history.
• Select nursing diagnoses and evaluate medications for adverse effects.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physicians of assessments and recommended interventions.
• Implement nursing interventions and reassess patient needs.
• Document progress and discharge notes in timely manner.
• Notify physician of changes in patient condition.
