Job summary

HCA Florida Gulf Coast Hospital is seeking a dynamic RN Case Manager to coordinate interdisciplinary care, evaluate clinical care necessity, and promote optimal resource use while supporting patient throughput. The role emphasizes collaboration with the care team to improve outcomes and ensure appropriate care levels.

The position requires Florida RN licensure, BLS, and 3 years of acute care RN experience, with preferred Case Management certification and BSN.

Qualifications

• Florida RN license required or eligible for multistate license.
• BLS-AHA Provider card required within 30 days of start.
• 3 years RN experience in acute care required.
• Prior Case Management experience preferred.

Responsibilities

• Perform comprehensive psychosocial and medical assessments.
• Advocate for patients and lead interdisciplinary team.
• Coordinate plan of care.
• Communicate with physicians, patients, families, hospital staff, and outside agencies.
• Perform utilization management reviews and inform third party payors.
• Support patient throughput with focus on quality and efficiency.
