Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge and transition planning, and manage outpatient care under supervision, ensuring appropriate care levels and providing education and decision support.

You will collaborate with a Care Advocate and Social Worker, participating in ongoing professional development and clinical programs to support patient outcomes.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ year of clinical experience as an RN.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and helps develop and implement a plan of care.
• Makes referrals as needed and documents rationale.
• Promotes patient independence through education and self-care coaching.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical team.
