UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, and support discharge planning. You will work under supervision of an RN or MD to ensure appropriate care levels, educate patients, and collaborate with a Care Advocate and Social Worker.

The role involves team collaboration, care planning, and ongoing education to improve patient outcomes across home health and outpatient settings.

Qualifications

• Current RN license valid in the state(s) you are assigned to work.
• 1+ year of clinical experience as an RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilités

• Performs patient assessments and develops a care plan with the care team.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes health and independence through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call duties as assigned.
• Participates in clinical development and continuing education.
• Other duties as assigned.
