Doctors Hospital of Augusta, part of HCA Healthcare, is seeking an experienced Registered Nurse (RN) Case Manager to guide patients and families through the entire care journey. You will coordinate services, develop care plans, and collaborate with physicians and staff to ensure safe transitions and consistent quality of care.

Qualifications

• Must be a graduate from an accredited school of Nursing: ADN or higher.
• Must be licensed as a Registered Nurse in the practicing state.
• 1–3 years of clinical, hospital nursing experience.
• 2 years in Case Management preferred.
• Certification in Case Management or Utilization Review preferred.
• InterQual experience preferred.

Responsabilités

• Coordinate plan of care for the patient stay and discharge planning.
• Facilitate progression and transition of care between levels of care.
• Perform psychosocial, medical and discharge needs assessments; reassess as needed.
• Ensure post-acute medical needs and level of care are appropriate with Social Services.
• Evaluate progression of care using evidence-based tools and approved criteria.
• Refer to third-party payer and disease/case management programs.
• Liaise with physicians, patient/family, hospital staff, and outside agencies as an advocate.
