Job summary

HCA Healthcare Trident Hospital in Charleston, South Carolina, is seeking an RN Case Manager to guide patients and families through care transitions. You will coordinate services, develop individualized care plans, and collaborate with clinicians to ensure safe, coordinated care.

The role requires an ADN or higher, RN licensure, 1–3 years of hospital nursing experience, and preferred CM/Utilization Review certification. A focus on collaboration and advocacy is essential.

Qualifications

• Must be a graduate from an accredited school of Nursing (ADN or higher).
• RN licensed in practicing state and/or active multi-state license.
• 1–3 years of clinical hospital nursing experience.
• 2 years in Case Management preferred.
• Certification in Case Management or Utilization Review preferred.
• InterQual experience preferred.

Responsibilities

• Coordinate plan of care for patient stays and discharge planning.
• Facilitate progression and transition of care between levels/settings.
• Conduct comprehensive psychosocial, medical, and discharge assessments.
• Partner with Social Services to ensure appropriate post-acute care.
• Communicate effectively with physicians, patients, families, and hospital staff.
