Job summary

HCA Healthcare Trident Hospital in Charleston, SC is seeking an RN Case Manager to support patients and families through every step of care. You will coordinate services, develop care plans, and collaborate with physicians, nurses, and department leaders to ensure safe transitions and quality care across the continuum.

You will assess needs, facilitate transitions, and ensure appropriate post-acute care in partnership with Social Services.

Qualifications

• Must be a graduate from an accredited school of Nursing: Associate’s Degree in Nursing or higher is required.
• Must be licensed as a Registered Nurse in the state which you practice and/or hold an active Compact/Multi-State RN license.
• 1-3 years of clinical, hospital nursing experience is required.
• 2 years of experience in Case Management is highly preferred.
• Certification in Case Management or Utilization Review is preferred.
• InterQual experience is preferred.

Responsibilities

• Coordinate the plan of care for the patient stay and discharge planning.
• Facilitate progression and transition of care and identify at-risk patients.
• Perform comprehensive psychosocial, medical, and discharge needs assessments for patients/families.
• Ensure post-acute medical needs and level of care are appropriate with Social Services.
• Evaluate progression of care using InterQual criteria and escalate issues as needed.
• Make referrals to third-party payer, disease and case management programs.
• Act as a liaison between physicians, patients/families, hospital staff, and outside agencies.
