AccentCare, Inc. is seeking an RN Case Manager to oversee patient care plans from admission through discharge in Madison County and North Jackson, MS. The role blends direct and indirect care, coordinating with staff nurses, LPNs, and CHHAs to deliver the plan of care.

This full-time position offers a schedule from Monday to Friday, 8:00am to 4:30pm, with on-call responsibilities. Pay ranges from $71,000 to $79,000 annually, reflecting an estimated compensation model.

Qualifications

Graduate from an approved school of professional nursing and licensed as RN in the state of operation.
Minimum of one year of RN experience.
Current CPR certification required.

Responsibilities

Assess status of nursing needs and manage patient care plans.
Complete initial admission assessment with patient history.
Select nursing diagnoses and monitor medications for safety.
Establish goals aligned with diagnosis and inform physician of interventions.
Implement nursing interventions within scope of practice and re-evaluate as needed.
Document assessments, progress, discharge notes timely.
Notify physician of changes in patient condition.
