Job summary

Covenant Health HomeCare is seeking a Registered Nurse to provide professional nursing care to homebound patients across East Tennessee. You will coordinate care planning with multidisciplinary teams and supervise LPNs and Home Health Aides as assigned.

Responsibilities include conducting comprehensive assessments, planning and revising care plans, administering medications, and teaching patients and families.

Qualifications

• Graduation from an accredited school of nursing.
• 1 year of professional hospital or home health experience preferred.
• TN RN license required; CPR required; valid TN driver's license with insurance; driving record must meet Covenant Health standards.

Responsibilities

• Provide professional nursing care to homebound patients and coordinate care planning with other disciplines.
• Perform cardiovascular, pulmonary, neurological, GI, skin, and pain assessments; maintain accurate documentation.
• Administer medications and treatments per policy; ensure safety and infection control.
• Lead case management within assigned caseload and act as liaison with physicians and families.
