Qualifications

Current RN license in good standing.
Minimum 1 year of home health or related experience with strong assessment skills.
Excellent organizational and time management skills.
Proficient with computer and internet-based applications and office equipment.
Competent working knowledge of client-based equipment.
Pass criminal background check and sanctions screening.
Professional appearance and demeanor.

Responsibilities

Manage nursing care coordination for home health and therapies after DOL approval.
Maintain confidentiality of all records in line with HIPAA and Privacy Act PHI.
Facilitate effective communication among the care team and families.
Perform other duties to support the team and patient care.
