Job summary

Chapters Health System seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care in patients' homes.

The role requires active RN licensure, home visit ability, and experience with EMR/EHR systems, with a focus on maintaining patient comfort and family support in a home setting.

Qualifications

• Active RN license in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• Completion of Alzheimer's Disease and Related Dementias Training for PACE employees (Florida).
• Experience with an EMR/EHR system.

Responsibilities

• Provide direct patient and family care as part of the Interdisciplinary Team (IDT).
• Coordinate the Plan of Care to reflect evolving patient needs.
• Perform home visits and assess patient conditions in home environments.
• Communicate with IDT and participate in care planning and referrals.
• Maintain SBAR-style clinical communications with the team.
