Job summary

Qualifications

• Must be able to assess clinical, psychosocial and financial needs of patients.

Responsibilities

• Assess patients and document needs (clinical, psychosocial, financial).
• Apply InterQual criteria to determine level of service within 24 hours.
• Identify care plan, LOS estimates, and discharge plan with patient/family.
• Coordinate with physicians and nurses for plan adjustments and transitions.
• Monitor treatment plan and ensure timely transition/discharge.
• Escalate barriers to appropriate treatment or discharge when needed.
• Maintain knowledge of facilities/resources for patients and caregivers.
