CVS Health is seeking a Case Manager, Registered Nurse in Houston, TX to apply a collaborative, member-centered approach to assess, plan, coordinate, evaluate, and advocate for individuals and families. This role is hospital-based and offers a full-time schedule.

The ideal candidate has an Associate’s Degree in Nursing with 3+ years of clinical and care management experience, Texas RN licensure, and is open to multi-state certification.

Qualifications

• Associate’s Degree in Nursing is required, Bachelor’s preferred.
• 3+ years of clinical experience, preferably with Medicare and Commercial populations.
• 3+ years in care management, discharge planning, or home health coordination preferred.
• Active and unrestricted RN license in Texas required.
• Willingness to obtain multi-state RN licensure preferred.
• National certifications (CRC, CDMS, CRRN, COHN, CCM) preferred.

Responsibilities

• Serve as liaison between members, families, providers, and healthcare personnel.
• Implement and coordinate case management for catastrophic and chronically ill members.
• Interact with members telephonically and in person during hospital visits.
• Assess medical, functional, and vocational status to develop care plans.
• Communicate with providers and insurance carriers.
• Prepare documentation of case management activities.
• Collaborate with multidisciplinary teams to support outcomes.
• Conduct outreach to treating physicians and specialists for care planning.
• Provide education and preventive guidance to promote outcomes.
• Monitor progress and adjust care plans as needed.
