The ideal candidate has BSN or higher, an active RN license, 1+ year in case management, and proficiency with EHR systems; multiple shifts are available, including day, night, and mid shifts.

Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ year experience in case management in healthcare or hospital setting.
• Strong interpersonal and communication skills to collaborate with medical professionals, patients, and families.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients including history, medications, treatment plans, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress, adjust care plans as needed and communicate changes to the team.
• Serve as main point of contact between patient and healthcare providers for seamless care coordination.
• Maintain up-to-date records and case notes, ensuring confidentiality and compliance.
