CVS Health is seeking a Case Manager, Registered Nurse to join a hospital-based care management team in Houston. The role focuses on assessment, care planning, coordination, evaluation, and advocacy for individuals and families, with a collaborative, member-centered approach.

The RN will work at Houston Methodist Hospital, coordinating services for catastrophic and chronically ill members, communicating with providers and insurers, and documenting all activities to support optimal patient

Qualifications

• Associate’s Degree in Nursing required, Bachelor’s Degree preferred.
• 3+ years of clinical experience, Medicare/Commercial populations preferred.
• 3+ years in care management, discharge planning, or home health coordination preferred.
• Active and unrestricted RN license in Texas required.

Responsibilities

• Serve as liaison between members, families, providers, and healthcare personnel.
• Coordinate case management activities for catastrophic and chronically ill members.
• Interact with members telephonically and in person at Houston Methodist Hospital.
• Assess medical, functional, and vocational status to develop individualized care plans.
• Communicate with medical providers and insurance carriers.
• Document all case management activities timely and accurately.
• Collaborate with multidisciplinary teams to support member outcomes.
• Outreach to treating physicians and specialists for care planning.
• Provide education and preventive guidance to promote outcomes.

What You'll Do

• Serve as a liaison between members, families, providers, and healthcare personnel.

• Interact with members both telephonically and in person at Houston Methodist Hospital.
• Assess medical, functional, and vocational status to develop individualized care plans.
• Communicate effectively with stakeholders including medical providers and insurance carriers.
• Prepare timely and accurate documentation of all case management activities.
• Collaborate with internal multidisciplinary teams to support optimal member outcomes.
• Conduct outreach to treating physicians and specialists for care planning.
• Provide education and preventive guidance to promote best clinical outcomes.
• Monitor member progress and adjust care plans as needed.

Requirements

Education:

• Associate’s Degree in Nursing required, Bachelor’s Degree preferred

Experience:

• 3+ years of clinical experience, preferably with Medicare and Commercial populations
• 3+ years of experience in care management, discharge planning, or home health coordination preferred

Licenses & Certifications:

• Active and unrestricted Registered Nurse (RN) License in Texas required
• Willingness to obtain multi-state RN licensure preferred
• National certification (CRC, CDMS, CRRN, COHN, or CCM) preferred
