Job summary

CVS Health's ACC Case Manager role is a hospital-based, on-site position supporting members at Houston Methodist Hospital. You will assess, plan, coordinate, evaluate, and advocate for comprehensive health needs across medical, behavioral, and social determinants of health.

The role requires Texas RN licensure, 3+ years of clinical experience, and strong analytical and communication skills. Travel requirements include consistent on-site presence to coordinate care with providers and teams.

Qualifications

• Active and unrestricted RN license (Texas).
• 3+ years of clinical experience, preferably with Medicare and Commercial populations.
• Ability to work independently within a hospital-based environment.
• Strong analytical, problem-solving, organizational, and communication skills.

Responsibilities

• Serve as liaison between members, families, providers and insurers to coordinate care.
• Coordinate and implement case management activities for complex cases across the continuum of care.
• Interact with members telephonically and in person, on-site at Houston Methodist Hospital.
• Prepare timely documentation of all case management activities and advocate for members.
• Collaborate with multidisciplinary teams to support optimal outcomes and timely return to work.
