Hospice of Virginia is hiring an RN Case Manager to support Forest and surrounding counties. Come join our compassionate growing team of professional clinicians who make a difference in the lives of patients and families facing end of life journey.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors impacting needs and coordinate with interdisciplinary team.
• Initiate communication with attending physician and interdisciplinary team to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.
