VHS Hospice is seeking a Hospice RN Case Manager to coordinate and manage the care of hospice patients across multiple Hampton Roads locations. The role offers full-time, part-time, and PRN options with daily pay and competitive hourly wages.
Responsibilities include developing individualized care plans, educating patients and families, supervising team members, and ensuring compliant documentation. Virginia RN licensure and prior case management experience are required.

$35-$45/hour (all inclusive)
