Job summary

Heart to Heart Hospice in Indiana is seeking a dedicated RN Case Manager to deliver compassionate in-home hospice care to patients with life-limiting illnesses. You will perform assessments, develop and adjust the care plan, and coordinate with IDG members to ensure quality patient outcomes.

Requirements include RN license, nursing degree, valid driver's license, and 2+ years RN experience preferred. We offer competitive pay, PTO, holidays, 401k, and a company car for qualifying individuals.

Responsibilities

• Conduct initial and ongoing assessments of patients’ needs and ADLs.
• Develop and implement individualized care plans and revisions.
• Coordinate care with physicians and interdisciplinary team; participate in IDG meetings.
• Provide on-call coverage and ensure timely documentation.
• Ensure compliance with regulations and organizational policies.
