Job summary

Moments Hospice in Wisconsin is seeking an RN Case Manager to lead clinical care for patients and families, assess needs, coordinate services, manage symptoms, and guide families through end-of-life care with compassion. This full-time, field-based role offers a Monday–Friday schedule and a supportive team.

Our fleet car program provides a company vehicle with gas and insurance covered, enabling you to deliver care where it’s needed most.

Qualifications

• Active RN license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• Compassionate, patient- and family-centered approach
• Valid driver's license and a reliable vehicle with current insurance

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
