Job summary

Interim HealthCare of the Twin Cities is seeking a Hospice RN Case Manager to provide individualized, coordinated hospice care in patients' homes within the Minneapolis area. You will assess needs, develop plans with the IDG, and ensure continuous, compassionate care.

This role offers independence in the field, a full benefits package including medical/dental/vision, PTO, a company car program, and reimbursement for licensing, CEUs, and travel.

Qualifications

  • Licensed to practice as an RN in MN and in good standing.
  • 12 months of RN experience within the last 5 years (or recent clinical knowledge as appropriate).
  • 1 year of hospice care experience preferred.
  • Travel within service area; reliable transportation, valid driver’s license, and proof of auto insurance.
  • CPR Certification.

Responsibilities

  • Completes initial/comprehensive assessments of patient/family to determine hospice needs and pain management.
  • Performs ongoing assessments of patient physical and psycho-social status.
  • Initiates and develops a care plan in collaboration with IDG, patient/family, and attending physician.
  • Coordinates care among IDG members and external providers to implement the plan of care.
  • Administers medications and treatments as prescribed, following nursing standards.
  • Provides education to support pain management and monitors response to treatment.
  • Directs services provided by LPN and Hospice Aide; supervises as needed.
  • Documents patient/family services accurately and timely.
  • Attends Interdisciplinary Group meetings.

As a Hospice RN Case Manager you will be providing individualized,coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional and spiritual well-being. You will have independence and flexibility in the field to visit patients one on one in their home environment.

  • Annual Salary Range: $89,419 - $106,620

A few must-haves for this position
  • Licensed to practice as an RN in the state of MN. License is active and in good standing.
  • Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate.
  • One (1) year of hospice care experience preferred.
  • Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.

Some functions of what you would do as a Hospice RN Case Manager
  • Completes an initial/comprehensive assessment of the patient/family to determine hospice needs which includes a history of current and previous illness(es), as well as a physical assessment of the patient, with emphasis on the management of pain and other symptoms.
  • Completes ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family.
  • Initiates and develops a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician.
  • Primarily responsible for the coordination of care among IDG members and other healthcare providers external to the IDG, to ensure the continuous assessment of the patient/family needs, as well as the implementation of the IDG plan of care to guide the delivery of services and achieve the goals of hospice care.
  • Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
  • Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment.
  • Directs the services provided by and supervises the LPN and Hospice Aide assigned to the patient/family.
  • Completes accurate and timely documentation of patient/family services provided.
  • Regularly attends Interdisciplinary Group meetings.

Hours

Full-time | Days | General working hours would be Monday - Friday from 8:00am - 5:00pm | Will also be a part of an On-Call rotation.

Territory

The RN will have a general territory within our service area where they will provide in-home hospice care to the patients. The territory for this position would primarily be in the southwest section of the Twin Cities Metro area.
