Job summary

Interim HealthCare of the Twin Cities seeks a Hospice RN Case Manager to provide individualized, in-home hospice care to patients and families, focusing on safety, comfort and holistic well-being. You will have independence and flexibility in the field to visit patients one on one in their home environment.

The role is full-time, with days and an on-call rotation, serving a southwest Twin Cities metro territory.

Qualifications

• Licensed to practice as an RN in MN with active license.
• Minimum 12 months RN experience in last 5 years.
• 1 year hospice care experience preferred.
• Valid driver's license, reliable transportation, auto insurance.

Responsibilities

• Complete initial/comprehensive patient/family assessment for hospice needs.
• Conduct ongoing physical and psycho-social assessments.
• Develop a care plan with IDG, patient/family, and physician.
• Coordinate care among IDG and external providers.
• Administer medications and treatments per physician orders.
• Provide education and interventions for pain and symptom management.
• Direct and supervise LPN and Hospice Aide services.
• Document services accurately and timely.
• Attend Interdisciplinary Group meetings regularly.

Job description

As a Hospice RN Case Manager you will be providing individualized, coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional, and spiritual well‑being. You will have independence and flexibility in the field to visit patients one on one in their home environment.

Hours

Full-time | Days | General working hours: Monday – Friday, 8:00 am – 5:00 pm. You will also be a part of an On‑Call rotation.

Territory

The RN will have a general territory within our service area, primarily in the southwest section of the Twin Cities Metro area, where they will provide in‑home hospice care to patients.

Pay & Benefits

• Annual Salary Range: $89,419 – $106,620

A few must‑has for this position

• Licensed to practice as an RN in the state of MN. License must be active and in good standing.
• Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge if no experience within the last five years.
• One (1) year of hospice care experience preferred.
• Ability to travel within the geographic area serviced by the office. Reliable transportation, valid driver’s license, and proof of auto insurance required.

Functions of a Hospice RN Case Manager

• Complete an initial/comprehensive assessment of the patient/family to determine hospice needs, including history of illnesses and physical assessment focusing on pain and symptom management.
• Conduct ongoing assessments of the patient’s physical status and psycho‑social status.
• Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician.
• Coordinate care among IDG members and external healthcare providers to ensure continuous assessment and implementation of the IDG plan.
• Administer medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
• Provide education and interventions to support effective management of pain and other symptoms, assessing the patient/family’s response.
• Direct and supervise services provided by the LPN and Hospice Aide assigned to the patient/family.
• Complete accurate and timely documentation of patient/family services provided.
• Attend Interdisciplinary Group meetings regularly.
