Interim HealthCare of the Twin Cities is seeking an RN Case Manager to deliver in-home assessments and care coordination across the southwest Twin Cities metro. The role offers independence, a field-based practice, and ongoing interdisciplinary collaboration.
Requirements include MN RN license, ability to travel, CPR, and 1–2 years nursing and case management experience. Competitive salary from $89,419 to $106,620 with generous PTO, health insurance, 401k, and professional development support.
The Registered Nurse (RN) Case Manager – Home Care will enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager will provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Hours: Full-time | Days | Monday - Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.
This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the southwest section of the Twin Cities Metro area.
