Job summary

Pathways Hospice is seeking a Hospice RN Case Manager to join our team in Houston. This full-time role covers Northwest, Southwest and Southeast Houston, coordinating patient care, collaborating with physicians and the care team, and ensuring a high standard of quality and compassionate service.

As an RN, you will oversee the nursing care team, assess patients, educate families, and handle on-call rotations, weekends, and admissions.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR.
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly)
