Job summary

Pathways Hospice (Newest Start Up) in Houston is seeking a Registered Nurse (RN) for PRN coverage across the Northwest, Southwest and Southeast Houston areas. You will oversee nursing care, coordinate with physicians, and ensure compassionate, patient-centered home hospice services.

The ideal candidate holds a Texas RN license, hospice experience, and can work independently while collaborating with a care team.

Qualifications

• Valid RN license in Texas.
• Hospice experience (1 year minimum).
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR (preferred).
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and families on managing health conditions, medications, and lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.

Hospice Registered Nurse (RN)

Service Area for Coverage: Northwest, Southwest & Southeast Houston

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
