Job summary

Pathways Hospice is seeking an experienced RN Case Manager to lead nursing care coordination across the Houston area. The role requires organizing home health services, developing patient care plans, and collaborating with physicians and therapists to deliver comprehensive care.

The ideal candidate holds a Texas RN license with hospice experience, excellent communication, and the ability to work independently within a team. Night/weekend coverage and on-call rotation are part of the schedule.

Qualifications

• Valid RN license in Texas.
• Hospice experience (1+ year).
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.
• If working the weekend-will receive the following Monday off (Admissions Included).
• Must be willing to participate in an On Call Rotation (Weekly Rotation).
