Job summary

Traditional Home Health Care and Hospice seeks an experienced RN Case Manager to provide hospice and community liaison services. You will assess patients in their homes across Dunmore, PA, develop care plans with physicians, and coordinate with the health care team.

Responsibilities include admissions, ongoing assessment, care plan management, and thorough documentation. Travel to assignments and strong communication skills are essential for success.

Qualifications

• Current license as a Registered Nurse in Pennsylvania is required.
• BLS certification is required.
• Must have a valid driver's license.
• Ability to travel to case assignments and work in patient homes.

Responsibilities

• Admit patients to Traditional Home Health Care.
• Assess patient status and develop plan of care with physician and family.
• Administer and evaluate patient care per physician orders.
• Report status changes to physician and nursing supervisor; revise plan as needed.
• Maintain meticulous documentation including care plans and time points.
• Act as liaison with patients, therapists, and health care team.
• Inform patients of additional services and make referrals as needed.

Job Title: RN Case Manager Hospice/Community Liaison

Job Location: Our Registered Nurses see patients in the most ideal setting - the patient's home!

Territories / Coverage Areas: Dunmore, PA and surrounding areas

Shift: Full-time 8:30am to 5:00pm

As an RN with Traditional Home Health Care and Hospice, you are able to give each patient your undivided attention and the time they deserve as you care for them.

Our offices are located in Dunmore, PA.

Job Responsibilities Include, But Not Limited To The Following

• Admits patients to Traditional Home Health Care.
• Assesses patient status and develops plan of care in collaboration with physician, nursing supervisor, patient and family members.
• Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with physician orders.
• Reports patient status changes, care, and progress to physician and nursing supervisor on a continuous basis. Revises plan of care by consultation with physician.
• Timely and accurately prepares, maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients, case conferences, medication changes, various OASIS time points, and changes to plan of care.
• Acts as a liaison and maintains contact with patients, therapist, nursing supervisor, and other members of the health care team.
• Informs patients of other services that are available and makes referrals to other community resources as needed.
  

Requirements

• Current license as a Registered Nurse in Pennsylvania
• BLS Certification
• Must have current driver's license
• Ability to travel to case assignments
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills
• Demonstrates good verbal and written communication, and organizational skills
• Acute care and/or home health experience is preferred.
